Angel is excited to treat you and looks forward to your first visit. As you prepare for your initial consultation and treatment you may have questions or concerns. Here are some of the more common topics and tips to help you get ready.
Have you filled out your forms yet?
Please print and fill out the following forms prior to your first appointment. Blank forms will be provided if you do not have a printer. Please come early to allow for this time if necessary.
- Intake form
- Health Information (Required for first treatment/and to be updated annually)
- Office Policies (Revised for 2022)
- Please arrive 7-10 minutes prior to your appointment
- Bring running/athletic shorts with you
- Do not take pain reliever (if possible) prior to treatment
- Drink plenty of water before and after treatment
- All forms of payments are accepted; I offer a $5 cash discount
- Please confirm your appointments via text when requested through Square
Parking is available in both the front and rear, however entrance is in the front of the building.
What is the cancellation policy?
If you need to cancel or reschedule for any reason, I require a 24 hour notice.
When should I arrive for my appointment?
Please arrive for your appointment approximately 10-15 minutes before it is scheduled. This allows for COVID-19 protocol and procedures as well as time to discuss treatment.
You should be on the table and ready for treatment at the scheduled time.
How much clothing should I remove for my treatment?
This varies based on both the patient’s comfort level as well as the type of therapy being administered. Feel free to share any concerns you may have with Angel.
Should I talk or be quiet when on the table?
It’s entirely up to you. Sometimes one may just want to unplug from the world. Others may prefer to connect in a safe environment. Angel is happy to follow your lead.
Clients can choose to remain silent throughout the treatment; however, Angel may seek feedback on pressure levels or sensitivity.
Should I tip my therapist?
This is entirely at the client’s discretion. Although they are greatly appreciated, they are certainly not expected.
What should I expect after my treatment?
